We know your time is valuable, to make check-in quick and efficient we ask
that you read over this information carefully.

1) Download and complete the following forms. These files provide necessary information about the care of your cat before and after surgery. You must complete both sides of the Surgical Release or your cat will not be admitted.
Also bring any prior veterinary records for your cat.

2) Have cash or money order ready. You must pay at the time of check-in.

3) Check-in starts promptly at 9 A.M. Please form a line in the order that you arrive. In inclement weather, leave your cat in the car while you check-in.
